February 19, 1917, Ronceverte, Greenbrier County-Conference with mayor and city officials regarding purification of city water supply; chlorinating apparatus to be ordered at once.

March 9, 1917, Cabin Creek, Kanawha County-Inspection of sanitary conditions now that houses are being rebuilt. Sanitary toilets being constructed in large numbers at Cherokee and shipped to all points on the creek.

April 13, 1917, Kenova, Wayne County-Inspection of water supply. Chlorination urged.

April 16, 1917, Summersville, Nicholas County-Conference with County Court in regard to disposal of sewage from the jail and other county buildings.

April 20, 1917, White Sulphur Springs, Greenbrier County-Interview with Mr. Meachem regarding clean-up of the town of White Sulphur. Inspection of some of this work which is being carried on by the boy scouts. April 23, 1917, Charles Town, Jefferson County-Visit to city spring. April 24, 1917, Martinsburg, Berkeley County-Conference with mayor in regard to the introduction of liquid chlorine in place of hypochlorite

April 25, 1917, Keyser, Mineral County-Careful inspection of city water supply, including visit to mountain spring.

April 26, 1917, Westernport, Md.-Conference with mayor in regard to

installation of permanent chlorinating apparatus. This town is still using an apparatus which I installed when I was resident engineer for the Maryland State Department of Health.

April 27, 1917, Piedmont, Mineral County-Conference with Mr. Carroll in regard to making a change in the type of chlorinating plant in use. The plant furnished by the Electro Bleaching Gas Company having persistently failed to operate, I advised Mr. Carroll to decline to make any payment on this..

April 28, 1917, Grafton, Taylor County-Conference with mayor and city clerk in regard to purification of city water supply. Immediate chlorination urged. Advised to raise money for filtration plant.

July 8, 1916, Shinnston, Harrison County-Detailed examination of the water system was made. Supply not used for drinking purposes. New safe drinking water supply advocated. Wells now furnishing supply show evidence of contamination.

July 10-11-12, 1916, West Union, Doddridge County-Stadia survey of West Union with view to planning new drinking water supply for the town.

July 12, 1916, Salem, Harrison County-Investigation of the public water supply. New pump station nearing completion at Patterson Fork. July 13, 1916, Broad Oaks, Harrison County-Nuisances above water plant on Elk Creek investigated. Conference with mayor and health officer regarding insanitary conditions and proposed clean-up campaign.

July 14, 1916. Broad Oaks, Harrison County-Insanitary conditions investigated at Broad Oaks.

July 14, 1916, Bridgeport, Harrison County-Investigation of public water supply at Bridgeport.

July 15, 1916, Northview, Harrison County-Public water supply investigated. New system at Northview not in operation. Inadequate supply of water.

July 17, 1916, Broad Oaks, Harrison County-Public address, "Clean-Up Campaign for Broad Oaks." Leaflets prepared by hectograph for distribution during campaign.

July 18, 1916, Stealy Heights, Harrison County-Conference with water company officials. Question of chlorination of supply discussed and company agreed to install plant to treat the water. Plant ordered at once.

July 19, 1916, Weston, Lewis County-Public water supply examined. Conference with manager of water company. Liquid chlorine treatment recommended to replace the present hypochlorite method.

July 20, 1916, Buckhannon, Upshur County-Investigation of water supply. Water company willing to co-operate for improvement of quality of water. Recommended that treatment by liquid chlorine be adopted immediately.

July 21, 1916, Sutton, Braxton County-Examination of public water supply.

July 25, 1916, Charleston, Kanawha County-Public address at semiannual meeting of county health officers on "Value of Chemical Examination of Water in the Interpretation of Bacteriological Analyses."

August 1-2-3-4, 1916, Parkersburg, Wood County-Typhoid fever epidemic investigation. Bacteriological analyses of water from suspected wells and from city supply, made at Dr. L. O. Rose's laboratory. Control measures taken.

August 5, 1916, South Parkersburg, Wood County-Investigation of insanitary conditions at houses near Pottery Works. Method of sewage disposal at pottery also examined.

August 6-7, 1916, Parkersburg, Wood County-Completed work in connection with typhoid fever outbreak. Preliminary investigation of Parkersburg supply.

August 9-20, 1916, Cabin Creek, Kanawha County; Coal River, Boone County-Emergency field work at time of flood. Bodies buried, wells cleaned and disinfected, typhoid inoculations given, typhoid vaccine distributed to mine physicians. Public address at Eskdale.

August 23-24, 1916. Eskdale, Kanawha County-Cabin Creek again visited to distribute typhoid vaccine to physicians along creek.

September 14, 1916, Ravenswood, Jackson County-Investigation of public water supply. School house visited for purpose of advising relative to changes in heating system. Insanitary conditions inspected and orders issued. Dysentery outbreak studied. Recommendations made to mayor for improvements.

September 15, 1916, Spencer, Roane County-City water supply examination. Nuisance from open sewer discharging near the Main Street investigated. Visit to State Insane Hospital and examination of sewage disposal plant and water supply for the institution. Recommended more attention be given the sewage disposal plant, which had been allowed to fall into disuse.

September 16, 1916, Ripley, Jackson County-Public water supply investigated. Conference with mayor in regard to improvements of water system and relative to investigating typhoid fever in the community.

September 28, 1916, New Martinsville-Typhoid fever outbreak studied. Due to infected well. Control measures taken. Public water supply examined. Conference with county health officer.

September 29, 1916, Grafton, Taylor County-Examination of the public water supply. Conference with mayor and council in regard to improvement by chlorination.

October 2, 1916. Fairmont, Marion County-Water Commissioner Mr. Ira Smith interviewed with regard to city supply and chlorination. October 2, 1916, Mannington, Marion County-Met city council at Mannington to discuss necessity for protection of their water supply.

October 4, 1916, Buckhannon, Upshur County-Chlorinating plant installed at the pumping station, Buckhannon Light and Water Company. October 5, 1916, Sutton, Braxton County-Conference with health officer in reference to improvement of the water supply by chlorination.

October 16, 1916, Eastbank, Kanawha County-Visit to advise in regard to construction work on the new water supply.

October 19-21, 1916, St. Marys, Pleasants County-Health exhibit shown at County Fair. Investigation of public water supply. Question of chlorination of supply discussed with city officials.

November 1, 1916, Clendenin, Kanawha County-Installation of chlorinating plant for public water supply.

November 6-8, 1916, Hill Crest, Charleston, Kanawha County-Topographical survey of grounds about Tuberculosis Sanitarium. Question of disposal of sewage investigated.

November 21-23, 1916, Martinsburg, Berkeley County-Study of methods employed by U. S. P. H. S. in rural sanitation work. Resurvey of Berkeley County being made by Drs. L. L. Lunsden and J. Sharp. Visits to Blairton quarries and operations of Pittsburg Limestone Company.

November 27, 1916, Welch, McDowell County-Public water supply examined. Operation of chlorinating plant satisfactory. Miners' Hospital No. 1 visited and investigation of water supply made. December 5, 1916, Weston, Lewis County-Chlorinating apparatus installed at Weston water plant. Hypochlorite treatment abandoned.

December 7, 1916, Philippi, Barbour County-Examination of the public water supply. Chlorinating plant out of order. Recommended immediate attention be given to put plant into operation since water is unsafe for drinking purposes unless disinfected.

December 8, 1916, Belington, Barbour County-Public water supply examined. Safe only portion of year. Question of chlorination taken up with manager. Favorable action promised.

January 3, 1917, Keyser, Mineral County-Investigation of public water supply. Spring in mountains visited. Also saw Mr. Patchett's well, where auxiliary supply is obtained. City council and doctors addressed in evening in regard to chlorination of supply.

January 4, 1917, Keyser. Mineral County-Investigation of water supply continued. Met adjourned meeting of council and action for safeguarding water supply promised. Council willing to spend $700 or $800 to protect water supply by chlorination.

January 5, 1917, Stealy Heights. Harrison County-Reinvestigation of water supply. New pumping equipment being installed.

January 9-17, 1917. Summersville, Nicholas County-Stadia survey of town to lay out new water and sewerage systems.

January 24-25. 1917. New Martinsville, Wetzel County-Typhoid investigation at request of city health officer. Cause of fever-milk supplied by certain dairyman. Steps taken to control spread of fever. Typhoid carrier at farm probable cause of outbreak.

January 26, 1917. Parkersburg. Wood County-Brief study of so-called "Smith System of Filtration." Many private houses visited to obtain data as to quality of water being supplied. Typhoid fever data for past two years obtained from city officers.

February 12, 1917. Clendenin, Kanawha County-Typhoid fever outbreak. Data obtained from all patients and cause of fever determined. Chlorinating plant was not operated for short time during very high water on Elk River, with result that contaminated water was pumped to city mains. School house inspection made. Sanitary conditions good.

February 15-16-17-18-19-20-21-22, 1917. Charleston, Kanawha CountySanitary survey work in connection with milk supply of the city. Majority
