Ed Gebhart has been in my office several times since September 15, 1943. He paid cash for the calls. There have been no payments or additions to the above bill since September 15, 1943.

We wish to make the following explanation showing why a mistake was made between the total number of days the patient spent in the hospital as shown on our statement, and the total number of days as shown on the patient's record. On September 8, 1940, the record shows 1,062 hospital days. The bedside notes were missing between September 8 and December 8, 1940. The record then shows 2,052 hospital days. There was here a difference of 990 hospital days, when the correct number should have been 90 (error of 900 days).

On June 28, 1941. the record shows 2,106 hospital days. On June 29, 1941, the record shows 3,008 hospital days (error of 902 hospital days).

On going through the record we found that on different occasions the student nurses made other small errors; in some cases they added days and in other instances they lost some days.

We trust this explanation is satisfactory.
